Referrals and Appointments
We accept new patient appointments preferably by referral from primary care physicians, therapists, educators and clergy. Providers will need to fax a referral on one of our referral forms along with medical records and insurance information to 864-676-9765 or 864-676-9432. Referral forms are available and can be printed from the Forms section on the home page. For more information, you can call our Referral Coordinator at 864-676-9211, ext. 137.
Established patients can schedule appointments by calling our appointment line at 864-676-9211, ext. 143, between the hours of 8am-4 pm Monday-Thursday, and 8 am-12 pm on Friday .
If you are unable to keep a scheduled appointment, please call 24 hours in advance to cancel or reschedule. Late cancellations or "no shows" are subject to charges.
